It is now well established that repetitive participation in vigorous physical exercise results in significant changes in myocardial structure and function. For most people, the development of a large heart is not a pathological sign rather, it is a desirable outcome that will enhance performance on the sports field, and will allow longer independence in old age. This review provides an insight of the science of cardiac remodeling in athletes and an overview of common clinical issues that are encountered in the cardiovascular care of the athlete.
